Cancelling Childcare Form

Parents/guardians wishing to cancel childcare must complete the form below in order to give written notice.

The form must be lodged with at least two weeks of care remaining. So if your child attends care on Mondays and cancellation is received on a Tuesday, the child should attend two more Mondays of care.

If you are receiving the Childcare Benefit then Department of Education, Employment and Workplace Relations (DEEWR) policy states that the child's physical attendance on the last day of care is required, if you do not attend you will not receive Childcare Benefit and full fees will apply. If your child is not well a doctors certificate can be submitted and the day will be an allowable absence. If you are going to attend another centre you cannot claim CCB from both centres simultaneously and would need to pay full fees at one centre.
